All paperwork for our ELC students must be updated once a year. If you would like to fill out paperwork now and mail it in to us, you would be able to register for summer classes without having to come in to the office. We need the Medical Form for your student (please make sure you put an email address for communications), the Assumption of Risk Form, and the Unlimited personal agreement (only for NEW students)
